【FRU Officers Fatal Accident】 Preliminary Report Indicates Seatbelt Absence and Overloading as Key Factors

(Kuala Lumpur, 12th) The Ministry of Transport today released a preliminary report on the accident that occurred in May this year in Teluk Intan, where a Federal Reserve Unit (FRU) truck collided with a lorry, tragically resulting in the deaths of 9 FRU officers. The report indicates that the main causes of this tragedy include the lorry's illegal overloading, non-compliance with vehicle technical standards, and a severe lack of safety equipment.
The report noted that the lorry overloaded with gravel had a total weight (BDM) of 40,960 kilograms at the time of accident, exceeding its approved load (24,000 kilograms) by 70.67%. This not only violated the Land Public Transport Agency's regulations but also significantly increased the vehicle's kinetic energy and impact force.
According to the technical analysis by the Road Transport Department, a vehicle weighing 24,000 kilograms traveling at 30 kilometers per hour generates about 833 kilojoules of kinetic energy; whereas a 40,960-kilogram vehicle moving at the same speed produces up to 1,422 kilojoules of kinetic energy. This enormous difference in kinetic energy, compounded by potentially higher speeds, greatly intensified the impact force during the accident, posing a fatal threat to individuals.
Furthermore, the investigation found that the involved lorry was only licensed to transport coal, with its side door height at 1,016 millimeters. However, vehicles carrying sand and gravel-type construction materials are not allowed to exceed a side door height of 762 millimeters, indicating non-compliance with the original approved technical specifications at that time.
Despite the lorry driver claiming a steering system malfunction, the technical inspection found no apparent faults in the steering system or other mechanical systems, including brakes, tires, and suspension. The only anomaly observed was an overheating of the drum brakes, likely due to prolonged frequent braking coupled with overloading.
The lorry had just passed a roadworthiness inspection at Puspakom on March 21, 2025, but had previously failed twice on September 24, 2024, due to brake inefficiency and imbalance, after which repairs were completed and it was re-certified on the same day.
As for the FRU truck that was struck, the report shows the rear seat had a 'bench-type' design, lacking any form of seatbelt, including for the driver and co-driver positions. The rear compartment's structure also did not comply with the United Nations vehicle safety regulations UN Regulation No. 17 or No. 80.
During the collision, most officers were thrown to the front left area inside the rear compartment, causing severe secondary impacts, including violent collisions between people and between people and compartment walls. Unsecured items inside the carriage became 'missiles' during the collision, further exacerbating injuries.
The report highlights that the lack of seatbelts and unsecured items in the compartment were key factors leading to the high fatality and injury rates. The Ministry of Transport calls for an immediate structural safety upgrade of similar government enforcement vehicles, including mandatory seatbelt installation, inspection of seat design, and enhancement of internal item securing mechanisms.

Of these, 8 died on the scene, and 1 died at Teluk Intan Hospital despite rescue efforts.
The Ministry of Transport suggests that the government reassess all public service vehicle safety standards, especially for transportation vehicles involving law enforcement personnel. It also urges private transportation operators to strictly adhere to load, transport purpose, and technical specifications to prevent the recurrence of such tragedies.
